About the UGFHT

What We Do

The Upper Grand Family Health Team (UGFHT) is comprised of a group of 27 family physicians at 8 sites and 24 allied health professionals who provide primary care services to approximately 26,000 patients in Fergus, Elora and Arthur. The UGFHT is funded by the Ministry of Health and Long-Term Care. The UGFHT allied health professionals consists of nurses, dietitians, social workers, pharmacist and health promoter which provide assessments, education and counselling services.
The UGFHT has been incorporated since January 2006 but many of our physicians have been serving the community for a number of years, providing care for patients from birth to old-age, delivering newborns to arranging palliative care. Physicians’ offices are located in all our centres: three in Elora, four in Fergus and one in Arthur. The Family Health Team has its main office in Fergus and shares facilities in Elora and Arthur.
As part of this Ministry of Health initiative, our purpose is to provide a broad range of health-care services that focuses on health promotion, disease prevention and chronic disease management, in addition to traditional medicine. Our goals are to enhance access to primary care, to decrease utilization of local hospital emergency departments and to increase the community and individual’s opportunities to improve health locally and for themselves. This increased capacity to meet a variety of community needs is to be supported by 22 allied professionals in mental health, psychiatry, pharmacy, dietetics, palliative care and nursing specialties. As part of this purpose, we seek to strengthen and deepen community partnerships with a variety of providers: The Waterloo Wellington Community Care Access Centre, Community Mental Health Clinic, Guelph, Wellington Dufferin Public Health, Groves Memorial Community Hospital and various other private and public groups, that will maximize resources and minimize waiting times.
The Upper Grand Family Health Team (FHT) is administered by a Board of Directors composed of the Upper Grand Family Health Organization (FHO) and community stakeholders. The executive director and the FHT physician lead report to the board, while the allied professionals report to the executive director. Our emphasis is on patients' feeling empowered and encouraged to play a central role in their health care and recovery. We are dedicated to achieving excellence in patient care and we foster the development of future healthcare professionals through offering student internship and educational opportunities with our organization and our physician partners.
