SENIORS CARE

 

The goal of this service is to help seniors enjoy an enhanced quality of life and to promote independence as well as to provide education in:

  • Falls prevention
  • Gentle exercise
  • Healthy eating
  • Medication management
  • Supporting a positive outlook on life
  • Specialized equipment
  • Other available services

Education is achieved through an individual nurse assessment and recommendation. After the assessment, referrals to other programs such as the Memory Clinic, Geriatrician, or Psycho-geriatric Clinic may occur.

A referral from your family physician or health care provider is required.

Memory Clinic

Duration: 2.5 Hour 

Registration: Referral required from family physician

An interdisciplinary assessment for adults who specifically have concerns regarding their memory.

Geriatric Specialists

Duration: Initial Appointment 1.5 Hour

Registration: Referral required by family physician 

Initial assessments take place with our Geriatric Nurse Specialist. A referral is then sent to the Geriatrician or Geriatric Psychiatrist for a consultation on diagnosis and/or medications. The nurse continues to partner with you in your specialized geriatric care, as required. Geriatrician assessments are offered via OTN or in-person. Geriatric Psychiatry is only offered via OTN.

Geriatric Team Assessment

Duration: 1.5 Hours

Home or office visits with patients and care givers. Comprehensive geriatric assessments with team-based approach. Team members may include: Geriatric Social Worker, Occupational Therapist and Geriatric Nurse. Pharmacy Consultations may also be included as part of the care plan.
