An estimated 600,000 Canadians live with heart failure. Heart failure (or Congestive Heart Failure, CHF) is a common condition that can develop after the heart becomes weakened, most commonly as a result of a heart attack or uncontrolled high blood pressure. It is a progressive disease. (Heart & Stroke Foundation of Canada, 2018)
Patients can receive treatment and make lifestyle changes that will slow the progression of the disease, can reduce or eliminate some symptoms, improve the quality and length of their lives.

Individual Consultations
We provide one-on-one consultations to include ongoing care, consultation and education/counseling:
SERVICES:
- Initial assessment and ongoing evaluation/treatment of condition
- Individualized patient and family education about the disease process, non-medical self-management strategies, self-monitoring and how to handle symptom flare-ups
- Medication review and dose adjustments in coordination with Physician and/or Cardiologist
