Heart disease is a general term that means the heart is not working properly (Heart and Stroke Canada, 2023). There are many types of heart disease that can help you with including hypertension, heart failure (congestive heart failure or CHF), or high cholesterol.

Our team can help you explore lifestyle changes and treatment options so you can manage your condition and lead a full and normal life. Meet with a chronic disease management nurse or registered dietitian to learn about heart health.

SERVICES:

  • Individualized patient and family education about the disease process, non-medical self-management strategies including nutrition and movement, self-monitoring and how to handle symptom flare-ups
  • Medication review and dose adjustments in coordination with Physician and/or Cardiologist

HOW TO BOOK AN APPOINTMENT:

  • To access this service, ask your primary care provider (family physician or nurse practitioner) for more information and to request a referral.
  • Referrals from your primary care provider are preferred, but if this is not an option, please call the Family Health Team at 519-843-3947, and we can talk to you about getting started.
