The Upper Grand Family Health Team staff at the Fergus site (753 Tower St. S) will be relocating to our Elora site located at 143 Metcalfe St. on November 16th, 2018.

Our offices will be closed:

Friday November 16th

Monday November 19th

Reception will be located on the second floor for patients to register, when our office re-opens on Tuesday November 20th

Our clinical team consists of nurse specialists, dietitians, mental health therapists, nurse practitioners, health promoter, pharmacist, occupational therapist and physician assistant who are supported by administrative staff to serve the residents of Centre Wellington. We work closely with our affiliated physicians to ensure that patients are well-supported to achieve their healthcare goals.
