Who We Are

The Upper Grand Family Health Team provides a broad range of health-care services that focus on health promotion, disease prevention and chronic disease management.

Our clinical team consists of nurses, dietitians, mental health therapists, nurse practitioners, kinesiologists, pharmacists, occupational therapists and a physician assistant who are supported by administrative staff to serve the residents of Centre Wellington. We work closely with our affiliated physicians to ensure that patients are well-supported to achieve their healthcare goals.
