Chronic Pain Management is an integrated care approach to managing illness which includes regular check-ups, consultations, monitoring and coordinating treatment and patient role education.
Individual Consultation
One-on-One Consultations are offered to any patient wishing to understand more about their chronic pain and how to best manage it. Chronic Pain Assessments are done by a multidisciplinary team with a focus on education of the neurophysiology of pain, self-management strategies, and recommendations for medication optimization when appropriate. The Assessment typically takes an hour.
