Although the risk of transmission in Centre Wellington is low, the Upper Grand Family Health Team is taking the proactive measures listed below to limit the transmission of the Novel Coronavirus (COVID-19) among our patients, staff and in our community.

*NEW* TEMPORARY DISCONTINUATION OF HOME VISITS AND IN-PERSON APPOINTMENTS
Beginning March 16, 2020 FHT staff working out of our main office location at 143 Metcalfe Street in Elora will be postponing home visits and providing appointments via phone only. This includes our Geriatric, Palliative and Chronic Disease Management Nurse Specialists, Geriatric and Palliative Social Workers, Mental Health Therapists, Occupational Therapist, Addictions Counselors, Pharmacist, Health Promoter and Dietitians. Access to these services will continue to be available to patients who are referred by their family physician. Appointments requiring in-person sessions will be rescheduled once current COVID-19 precautionary measures are no longer in place.
