The Supportive and Palliative Care Program provides nursing and social work support to patient and their families as they navigate serious illness, end of life and bereavement.
This program strives to ensure that patients feel supported as they navigate systems and cope with their illness. The Nurse and Social Worker work collaboratively with the healthcare team to provide pain and symptom management, access to resources including financial supports, end-of-life planning and anticipatory grief and bereavement counselling.
The program strives to ensure the best care for the patient and their caregivers.
Individual Consultation
SERVICES:
- Provide home, hospital or office visits
- Patient assessments
- Pain and symptom management
- System navigation and referral to services
- Consultation with the health care team, including family and palliative doctor
- Collaboration with all members of healthcare team
- End-of-life planning
- Anticipatory Grief and bereavement counselling.
