PALLIATIVE & SUPPORTIVE CARE

 

Palliative and Supportive care services are for those with a life threatening or palliative diagnosis.

This program strives to ensure that patients have access to appropriate and timely services in a seamless manner. They can provide help with pain and symptom control, accessing resources and financial help, counseling, as well as grief and bereavement support. The goal of these services is to decrease the number of emergency department visits for palliative patients related to a health care crisis or pain control, and increase the number of deaths at home for those palliative patients who choose to die at home. Palliative and supportive care seeks to enhance communication and collaboration among “end of life” agencies and providers through patients’ disease progression.

Individual Consultation

Our Palliative Care Team, consisting of a palliative care nurse specialist and social worker, can provide support, coping mechanisms, education and assistance to patients and their families dealing with terminal illness, end stages of life and following death.

SERVICES:

  • Home visits: patient assessments, pain and symptom management, physician consultations, collaboration with community partners for services
  • Home, hospital and office visits: end of life and bereavement counseling, and wellness counselling for clients with life threatening illness and their families.
  • Caregiver support groups
